Required Forms for Volunteer Service

WHERE TO SEND YOUR PAPERWORK: To apply for volunteer service, download and complete the following forms, then mail, fax, or email as attachments to your volunteer Coordinator. If you do not have a School Coordinator, please send forms to the North American Division Office at FAX 301-680-5079 or mail to Ernest Hernandez, Volunteer Processing Coordinator 12501 Old Columbia Pike, Silver Spring, MD 20904.
SHORT-TERM VOLUNTEERS - individual assignments of less than 2 months
RETURN VOLUNTEERS - previously processed through NAD/GC Office
Less than one Year
More than one Year but less than 4 years
- Only One Reference Form
- No PPM (Preparation for Missions course)
- Everything else that is required for First Time Volunteers
FIRST TIME VOLUNTEERS -processed through NAD/GC Office for the First Time.
Please complete the following 6 or 7 steps.
- Online Application
- CV/RESUME
- Beneficiary Form and either the Release of Liability Form or, if you choose to pay for the insurance premiums yourself, use the Self-Funded Release of Liability Form .
- Health Certificate - to be completed by a medical professional
- Reference Form - three references are required (your pastor and two professionals from your work or
school), and must be completed and sent by the three individuals listed on your Application form.
- PREPARATION FOR MISSIONS ORIENTATION is required for all international assignments and can be obtained through group training on an Adventist school campus or through the online short self-study course called Preparation for Mission. After submitting your online application, you will receive an email with a link to the course and an enrollment key. Write down the enrollment key (which will also appear on your submitted application's home page) and LOGIN to access this course. If you wish to purchase the book called Passport to Missions, please write to the Institute of World Missions at iwm@andrews.edu. The cost is $12 USD plus shipping and handling.
- For positions in Guam Micronesia Mission download, sign and fax the Volunteer Agreement to Guam.

INSTRUCTIONS FOR TRAVELING SPOUSES: If your spouse is going as a volunteer, he/she must be processed as an individual volunteer. A non-Volunteer spouse is required to complete the following:
1. Spouse Beneficiary Form
2 Spouse Release of Liability Form ,
3. Spouse Information Form ,
4. Spouse Declaration Form
NOTE: Married couples cannot be separated on assignments for more than 3 months per NAD/GC policy.
